| • Cyclophosphamide 200 mg/m2/day x 4 days |
| • Doxorubicin 12.5 mg/m2/day x 4 days |
| • Etoposide, 60 mg/m2/day x 4 days |
| • Intrathecal prophylaxis in patients with marrow involvement or small non-cleaved (Burkitt or Burkitt-like) histology |
| • Prophylaxis against opportunistic infections (co-trimoxazole, fluconazole) |
| • Granulocyte-colony stimulating factor |
