Requirements for release of a sibling donor cord blood unit from Children's Hospital Oakland Research Institute (CHORI) to a transplant center
| 1. Return of a signed copy of the medical director's CBU release letter to the CHORI Sibling Donor Cord Blood Program |
| 2. Confirmatory HLA typing prior to unit shipment |
| 3. Copy of the transplantation protocol to be used |
| 4. Copy of the transplant center's institutional review board approval for the transplantation protocol to be used |
| 5. Copy of the family's signed consent for transplantation on this protocol |
| 6. Copy of the standard operating procedure to be used for thawing if it is other than that recommended by the CHORI Sibling Donor Cord Blood Program |
| 7. Agreement to provide requested postthaw cell recovery and clinical outcome data in a timely manner |
