| Doxorubicin | 60 mg/m2 IV bolus over 15 minutes on day 1 |
| Vincristine | 1.5 mg/m2 (max 2 mg) IV days 1, 8, 15, 22 |

| PEG-L-asparaginase | 2500 IU/m2 IM randomized to weekly (days 1, 8, 15, 22) or every other week (days 1, 15) |
