At least one first-degree relative with a history of ischemic heart disease or sudden death before the age of 60 years.
Systolic blood pressure >140 mm Hg or diastolic blood pressure >95 mm Hg at the time of admission.
Weight more than 25% above the ideal body weight.
Total serum cholesterol level ≥200 mg/dL (5.2 mmol/L) at the time of admission for the cases and at the time of blood sampling for the controls.
