Histology Scoring System
Score . | Spleen . | Skin (Cheek) . | Marrow . | Liver . |
---|---|---|---|---|
0 | Normal | Normal | Normal | Normal (occasional lymphocytes around portal triads are acceptable) |
1 | Focal follicular infiltrates | Basal keratinocyte ballooning | Scattered mononuclear infiltrate | Rare (1 to 2/0.5 cm) focal collections of mononuclear cells in parenchyma) |
2 | >50% of white pulp follicles with infiltrate Red pulp infiltrate | Sebaceous and adnexal infiltrate | Aggregates of mononuclear cells | Endothelialitis present in one vessel/0.5 cm (Subendothelial infiltrate of a depth at least 2 cells in 1 vessel) |
3 | Effacement of splenic architecture | Loss of epidermis | Replacement of marrow by mononuclear infiltrate | Endothelialitis present in >3 vessels/0.5 cm with the infiltrating depth >3 cells |
4 | Not applicable | Not applicable | Not applicable | Endothelialitis as above present in virtually all vessels |

Spleen, skin, and bone marrow scoring used a 0 to 3 scale. Liver scoring used a 0 to 4 scale modified from that described by Blatter et al22 and James Crawford (personal communication, September 1996). 0, normal; 1, minimal; 2, mild and diffuse; 3, moderate damage; and 4, severe damage. The indices used were extent of endothelialitis, presence of apoptotic hepatocytes, infiltration of bile ducts, and focal collections of mononuclear cells. The maximum possible score is 13.
