International Working Group for Myelofibrosis Research and Treatment criteria for the diagnosis of post-PV MF

Required criteria
  1. Previous diagnosis of PV (WHO criteria)

  2. BM fibrosis grade 2 or 3 (on 0-3 scale)

 
Additional criteria (2 are required)
  1. Anemia* or sustained loss of requirement of phlebotomy or cytoreduction

  2. Leukoerythroblastic peripheral blood picture

  3. Increasing splenomegaly: palpable spleen > 5 cm from left costal margin, or appearance of a newly palpable splenomegaly

  4. Development of ≥ 1 of 3 constitutional symptoms (≥ 10% weight loss in 6 months, night sweats, unexplained fever > 37.5°C)
